We gladly inform you about the processing of your personal data.
We store your personal and medical data digitally, in order to provide you with good care. We process your personal data according to our privacy and data safety policy. You agree with this storage (processing) by having your care carried out by our practice. This is how we process your personal data:
- Your personal data is stored and used in order to provide you with good care.
- Your personal data is only shared with other parties in order to provide good care. When it is legally necessary to ask your permission first, we will ask this particular permission beforehand.
- Your personal data is processed in accordance with the treatment agreement as described in the ‘Medical Treatment Contracts Act’ (in Dutch: WGBO).
- You may see parts of your personal data. If you are interested in this, please send us a written request or use this form (Word document, in Dutch)
- You may request us by letter to adjust your personal data, when you think your personal data that we have stored is incorrect.
- You may request us to erase your personal data. When we do so, we are (or may) not be able to provide you with good care. The Dutch ‘Medical Treatment Contracts Act’ demands a holding period of 15 years. In that case we store your data in an inactive archive, invisible and unusable for normal users.
- You may withdraw your given permission regarding us processing your personal data. When you do so, we are (or may) not be able to provide you with good care any longer. In that case we store your data in an inactive archive (see above).
- You may object to the way we process your personal data.
- We will inform you when something goes wrong with your personal data.
- We store your personal data only for as long as necessary to provide you with good care. We do this according to the legal holding period stated by the ‘Medical Treatment Contracts Act’.
- Everything stated above also applies to personal data we receive from third parties.

Addendum: The self-triage service “Should I go to the doctor?
Our website uses the self-triage service “Should I see a doctor?” (hereinafter: the App), developed by MINDD B.V. in Apeldoorn. The App helps users determine whether contact with a doctor or GP is necessary. Because this involves the processing of personal data, we would like to inform you about how your data is handled.
Controller and joint processing:
The App is technically and content-wise managed by MINDD B.V.
Both MINDD B.V. and Huisartsen Ganzenhoef process personal data entered through the App:
· MINDD B.V. processes the data for the operation, security, analysis, and improvement of the App.
· Huisartsen Ganzenhoef receives the triage results to follow up on the user’s care request.
In this context, both parties are jointly responsible for the processing within the meaning of the General Data Protection Regulation (GDPR).
MINDD B.V. MINDD B.V. and Huisartsen Ganzenhoef have made written agreements about this in a collaboration agreement that defines the responsibilities for security, retention periods, and the rights of data subjects.
Which data is processed?:
When using the app, you enter personal data about yourself or another person. The app uses a unique session code (UID) so that not all data can be traced back to an individual.
The following data is processed via the app and is visible to both MINDD B.V. and Huisartsen Ganzenhoef:
· Gender
· Date of birth
· Answers entered during triage
· The outcome of the triage (urgency)
In addition:
· When you request a callback, additional data is processed: name, telephone number, address, GP, possibly a photo, and a description of your complaint.
· These callback requests are only visible to Huisartsen Ganzenhoef (the affiliated GP out-of-hours service or healthcare organization) and are automatically deleted after 72 hours. MINDD B.V. does not have access to the content of callback requests, but can technically verify whether the transmission was successful.
Purposes of processing
Personal data is processed for the following purposes:
· Performing triage and providing appropriate advice.
· Forwarding triage results to the affiliated healthcare organization for follow-up.
· Improving and analyzing the quality, safety, and performance of the App.
· Scientific research into the use and effectiveness of the App.
The data is not used for marketing purposes and is not shared with third parties outside the scope of healthcare provision or research as described below.
Scientific research:
MINDD B.V. conducts scientific research into the functioning of the App in collaboration with GP out-of-hours clinics and universities. Triage data is used for this purpose in anonymized form, making it impossible to identify individuals. Therefore, the research partners do not receive any personal data that can be traced back to a user.
Data security:
The App uses only secure (HTTPS) connections. Both MINDD B.V. Huisartsen Ganzenhoef has implemented appropriate technical and organizational measures to protect personal data against loss or unauthorized access.
All data is stored within the European Economic Area (EEA).
Retention Periods:
• Triage results: are retained by MINDD B.V. for as long as necessary for analysis and improvement of the App, in accordance with MINDD’s main privacy statement.
• Callback requests: remain visible to [client name] for a maximum of 72 hours and are then automatically deleted.
• Statistical and research data: are stored exclusively in anonymized form.
