You can arrange all your healthcare matters online via HuisartsDichtbij, without having to call the practice. The HuisartsDichtbij portal is accessible 7 days a week, 24 hours a day via our website or directly.

You log in with DigiD. Arranging things for your child? Then log in with your child’s DigiD. No DigiD yet? You can request this for any age via https://www.digid.nl/en/apply-or-activate-digid

You can get support on the website digital care helpdesk. Here you can find explanations about various questions about how you can find answers to your healthcare questions on the internet and our portal. You can also call the helpdesk on 085-1304575.

Make an appointment online:

You can schedule an appointment online with your GP via the HuisartsDichtbij patient portal.
The advantages:
• You are not on hold.
• Take a look at the empty spaces in the agenda and book an appointment at a time that suits you. We ask you to state the reason for your appointment in order to prepare the consultation.

Request a repeat prescription online:

• Go to HuisartsDichtbij.
• Log in with your DigiD.
• Go to the overview of your available repeat medication.
• Add the prescription you wish to request using the “Add” button.
• Click on “Request” to request the repeat medication.

Ask a question online:

• Go to HuisartsDichtbij.
• Log in with your DigiD.
• Click on e-consultation.
• Click on “ask a question”.
• Select the right care provider and describe your question.
• You can also add an image or document (maximum 10 MB) to your question.
• Click on “send” when all fields are filled in.
• When the healthcare provider has answered your question, the e-consultation will be closed automatically. You can then no longer ask a question in this e-consultation. To ask another question, it is best to start a new e-consultation.
• Usually your question will be answered within two or three working days.

Fill in self measurements:

It is possible to fill in selfmeasurements that you have carried out at home via HuisartsDichtbij. You can only pass on non-acute self measurements.

  1. Log in at HuisartsDichtbij using your DigiD.
  2. Select Measurements
  3. Choose the right measurement
  4. Fill in the detials
  5. Click on ‘Add measurement’
  6. When you have entered all the necessary self-measurements, click on ‘Send measurements to your GP practice’.

Online access to your file:

View your medical records at home. How do you do that?
Have you been to the doctor? Your GP puts your medical data in his computer in your medical file.

For example:
• The advice of the GP.
• The results of research.
• The list of your medicines.

You can read your medical data safely and calmly on your own computer, smartphone or tablet n the online portal HuisartsDichtbij. You will then have more information to talk to us about your health and to make a choice about your treatment.

About your self-measurements:

What are self-measurements?
With self-measurements you can independently and digitally add the following data to your file: blood pressure, blood glucose, heart rate and weight.

How can I add self-measurements?
You can add self-measurements if you have made agreements about this with your GP practice. Select ‘Self-measurements’ in the menu. Then click on “Add a measurement”. Choose which measurement data you want to add. Then enter the value, link the correct date (and time), and send any comments.

I cannot add self-measurements, how is that possible?
You can only add self-measurements if you have made an agreement about this with your GP practice. Your GP practice will give you access to the functionality within HuisartsDichtbij.

What happens to the self-measurements I sent?
The self-measurements are assessed by the GP practice and can be approved or rejected.

What happens to approved self-measurements?
After a check and approval by the GP practice, the values are added to your medical file. They can be seen in HuisartsDichtbij via ‘Results’. You can see measurements up to 14 months in the past.

What happens to rejected self-measurements?
When the GP practice rejects a self-measurement, the values are not added to your file.

Where can I view my self-measurements?

Will I also be called about my self-measurements?
